By Making a Payment through this payment channel you agree to the same Rules and Conditions as the Resident .

CRITERIA FOR ADMISSION:

1. Client must pass a urine analysis upon arrival.
2. Client must follow recommended treatment plan provided from client’s treatment team.
3. One time processing fee of $300 is do upon arrival to Choices Recovery.
4. One week’s rent must be paid upon arrival to Choices Recovery.

INTAKE PROCESS:

All clients who will be residing at Choices Recovery, LLC will complete an intake process with the House Manager, who is a Certified Behavioral Health Technician. This process will ensure that the client is provided with all the rules and regulations of Choices Recovery, LLC and complete all required paperwork. This includes signing consent forms and establishing short and long term goals.

CLIENT RESPONSIBILITIES:

1. Policies and procedures are enforced to create a safe and sober living environment.
2. Zero tolerance policy for non-compliance with facility rules.
3. To maintain a safe and supportive group atmosphere – teamwork is the key!
4. Follow the directives and advice offered by staff.
5. To report any changes in your program to the House Manager (i.e. change in sponsor or change in treatment plan).
6. To be respectful and considerate of the rights and properties of other clients.
7. To take responsibility for your personal space and the care of your own personal belongings.
8. Client is responsible for prescribed medications. (Must make house manager aware of ALL medications, including over the counter and any change to medications after arrival) No mind or mood altering substances allowed.

PROGRAM GUIDELINES:

• No use or possession of alcohol or any type of mood or mind-altering substance or paraphernalia. Infractions may result in discharge from program or referral to treatment or detoxification.
• Client will secure a sponsor within two weeks of residency.
• Client will attend 90 meetings in 90 days, followed by at least 4 meetings per week thereafter.
• Client will obtain a home group within two weeks of residency.
• All clients will attend mandatory 12 Step Meeting Tuesdays at 8:30 P.M. Female clients will also attend mandatory Women’s meeting Wednesdays at 6:45 P.M. for the first 90 days of residency.
• Client will attend mandatory weekly house meeting ran by the house manager.
• Client will secure employment within the first two weeks of residency.
• Client will be responsible for a weekly chore and keeping their bedroom and apartment clean daily. There will be no food or drinks allowed in the bedrooms. Client will make bed, put clothes away, wash dishes after use and keep their bathroom clean.
• Client will not take any medication prescribed to anyone else.
• Client will not give their medication to another client, under any circumstances.
• Client agrees to be submitted to a search of self and personal belongings as requested.
• Client will submit to drug testing on premises, including random drug testing.
• Theft will not be tolerated. Immediate discharge will occur.
• No violence or threats of violence of any kind, either physical or verbal. No weapons are permitted on property as well.
• Clients are to respect the property of fellow clients and that of Choices Recovery, LLC.
• Client will be responsible for the security of their own property.
• Smoking in to only occur outside of the facilities in designated smoking areas.
• No males on female properties. No females on male properties.
• Rooms and personal property may be searched at the staff’s discretion.
• Sober visitors are allowed on property up until curfew.
• CURFEW:
  • First week curfew is 10 P.M.
  • After first week curfew is 11 P.M. Sunday through Thursday. 1 A.M. Friday and Saturday.
  • Overnight passes are available for the use of clients after 30 days of residency. Client is to provide a written request 24 hours in advance describing the reason for the overnight and who they will be with.
    RENT:
    Rent is to be paid in full each Friday by 7 P.M. Client is to give the rent money to their house manager. Receipt will be provided upon request. If rent is late a $20 late fee will apply unless prior arrangements were made.

• Client must be out of bed and dressed by 9 A.M. Monday through Friday until employment is obtained. Client must be off property between 9:30 A.M. and 3 P.M. Monday through Friday as well until employment is obtained.

Administrative Fees:

A minimum non-refundable Administrative Fee of $300.00 is necessary in order to reserve a space. This administrative Fee is non-refundable under all conditions. If an Administrative Fee is paid through this or any payment channel to Choices Recovery and the intended resident does not take occupancy, the Administrative Fee is forfeited. 

Additional Rent:

All payments are applied first to any outstanding balances and then to any current amounts due. Rent is due on a weekly basis (i.e., from Friday to Friday) and is due on the Friday immediately preceding the following rental week. There are no partial refunds available for individuals leaving before the end of the rental week and/or prior to the full term of the rental agreement.

Client Funds:

The Client Funds Account is an additional service that Choices Recovery provides for our residents. All Payments for the Client Funds Account are subject to a convenience fee, which will be charged in addition to the amounts to be allocated to the individual client's cash account. Any credits will be refunded upon the completion of the terms of the Residential Lease, subject to any necessary cleaning charges and/or outstanding rent balances.

Partial Refunds:

There are no partial refunds available for individuals leaving before the end of the rental week and/or prior to the full term of the resident lease agreement. If a resident leaves our facility of their own decision all monies paid to Choices Recovery will be forfeited. If a resident breaks the terms of their lease agreement including the House Rules, all monies paid to Choices Recovery will be forfeited. Any refunds made will be credited within 30 days electronically to the card or bank account from which they originated. A Refund Notification will be forwarded to the email address that the Registered User (Payee) entered during registration.

Acknowledgement & Acceptance of Terms:

All Internet transactions are final, and are not subject to reversal and/or cancellation. All Internet payment transactions are subject to a Convenience Fee equal to $10.00. This Convenience Fee is disclosed and included in the total payment amount prior to submission of your payment for approval. Note regarding "Save this information" and our "Express Checkout" feature: Choices Recovery will not have access to this information. Your bank account and/or card data is encrypted prior to authorization. In exchange, the secure gateway returns a "token" to this payment system which can then be resubmitted with future payments in place of more sensitive information. End-to-End Encryption coupled with Tokenization ensures your data is secure.
